Patient Registration

Once you have spoken with one of our administrative staff and secured a new patient appointment, you may save time the day of your appointment by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Please assist us by providing the following information at the time of your consultation:

• Any x-rays taken by a previous dentist, if applicable.
• A list of medications you are presently taking.
• Your completed New Patient Forms, if not completed online
• If you have dental insurance, we will need this information ahead of time so we can verify coverage. This will save time on the day of your first appointment, help us provide the best estimate of coverage to you and allow us to  process any claims quickly and efficiently.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
